A loved one has dementia or memory loss – lots of changes occur when someone has dementia. Families are often at a loss about how to manage all of these changes. LifeLinks geriatric care managers can help evaluate the situation, educate the client and family and address concerns that come up throughout the disease progression.

There is conflict occurring within the family. By providing an independent assessment of the situation and being an outside, neutral professional, LifeLinks can dramatically reduce tension among family members. Our care managers can help make sure everyone’s voice is heard, but at the same time, remain focused on the older, vulnerable person to make sure their needs are met.

Mental illness and substance abuse is a factor (these problems may occur in the older person or even among family members). We help evaluate what’s going on and make recommendations about what to do to cope with the myriad of issues that can arise when mental illness or substance abuse is part of the problem.

Aging parent or family member refuses help, refuses to move, refuses to stop driving, or refuses to comply with medical advice. Often, there are reasons other than plain stubbornness at the heart of the problem. LifeLinks can help evaluate the reason for the resistance and develop a plan of action. While we don’t have a magic wand, we are often successful in developing an incremental strategy to help reduce harm to the client little by little. One of our strengths is our patience and ability to work through very difficult/complex problems.

Your aging loved one is at risk of harm. LifeLinks has the ability to thoroughly evaluate the situation and help determine ways to maximize independence, which is usually what the elder wants. By continuing to stay involved, the professional can adjust the plan of care over time to ensure that the client remains in the least restrictive setting as the situation changes.

Aging Family Services is now LifeLinks. The same care management staff in Raleigh that was Aging Family Services will continue the professional care upon which you’ve come to rely on as LifeLinks. Previous owner, Cheryl Theriault, will continue in the leadership role as the NC Triangle Area President and Amanda, Deb, Karen, Lisa, and Meike will all continue to care for our clients.
